Assessment

Assuring access to appropriate services is vital to successfully supporting people. 
Screening and evaluation are at the heart of all programs. 

Where is my assessment conducted?

Assessments are conducted in the outpatient offices at Sunrise Canyon Hospital, located at
1950 Aspen Avenue.  If you are unable to come to our offices, we can meet you at your home
or another location.
